An updated CORI CHECK is required for ALL wishing to volunteer or chaperone.
Please complete this form and contact Tanya who is a notary to set up a time to meet to provide a copy of your ID. Tayna can be reached at 508-493-7671.
Tanya will coordinate delivering completed paperwork to the school for submission.
